2025Jane Doe, age 77 was admitted to Woe Assisted Care in 2022. Doe was a fall risk and suffered a total of 18 falls, 8 between December 2, 2023 and January 4, 2024. Most falls were without injury. On January 4, 2024. Doe had her final fall when she lost her balance in the hallway, fracturing her left wrist and hitting her head with a resulting intracranial subdural hematoma, resulting in her death. The cause of her fall was disputed. The chart showed that a resident care director instructed the staff to assist Doe with walking with her walker to prevent falls exactly one year prior to her fatal fall. The case was resolved through mediation.
